Kenshi has never been particularly interested in making your life easy. You can get robbed, beaten unconscious, kidnapped, enslaved, or eaten, sometimes before you've accomplished anything more ambitious than walking to the next settlement. And apparently, for some players, that still wasn't quite enough.

Seth's Kenshi Kaizo takes the game's already unpredictable world and makes it even more reactive. Factions respond differently to changing circumstances, settlements can develop new political situations, and characters behave with a little more self-preservation and opportunism. Guards may rescue fallen allies, bandits can kidnap incapacitated victims, and some characters become recruitable through new conversations and situations. There are also changes to enemy encounters, world states, and faction campaigns that give returning players plenty of reasons to reconsider their usual strategies.

The overhaul is generally more demanding than vanilla Kenshi, and the creator recommends importing existing saves and placing the mod low in the load order. Compatibility with other major world-state overhauls, particularly Reactive World, can be troublesome without appropriate patches.

If you've already spent hundreds of hours learning how to survive Kenshi's particular brand of misery, Kaizo might be just the excuse you need to start making terrible decisions all over again.
